Please ONLY fill out this form if you are ACTIVELY looking for a puppy now or within around a year and would like to be considered for a spot on our current waiting lists. 

It form is used to help us better understand what your home environments and family lifestyles are like. This helps us tremendously in identifying if this breed is a good fit for your home, as well as, what the right personality, energy, and drive levels for your family would be. (We do have a separate contract/health guarantee for families to look over and sign once it is time to put down a puppy deposit.).
